Stress Hormones and Wellness

Cortisol is a stress hormone that tends to rise as we age. Excess cortisol production can lead to blood sugar problems. Cortisol is a corticosteroid. Synthetic versions of cortisol include Prednisone®. These synthetic versions are often times much more potent and while they can be useful to reduce inflammation they can also cause diabetes and weight gain.

Cortisol levels rise in your body in the morning and help energize your body. Your testosterone levels also rise in the morning at the same time. Drinking coffee stimulates cortisol production in your body and this is why it is generally okay to drink coffee in the morning but not in the afternoon. If your cortisol levels are elevated in the afternoon it can suppress thyroid function and prevent you from being able to go to sleep. In general, we like to measure cortisol 4 times during the day with an easy saliva test to figure out what your daily cortisol pattern is like. There are ways to increase and decrease cortisol levels as needed.

One aspect of women’s health that is often mistreated, in my opinion, is low mood. There are likely to be multiple factors contributing to the low mood and these can include hormone imbalances, relationship problems, and lifestyle. Prozac®, for example, can be helpful in certain situations but a deficiency of this medication is far from the cause of the problem. Some women may have a genetic need for more serotonin and there are many ways to improve these levels without the unwanted side effects that many experience from anti-depressants.

It is about what you do every day and NOT what you do once in a while (in general). With respect to diet, this means eating a healthy diet most days and still eating cake on your birthday. What is a healthy diet? We receommend the Paleo Diet.

The paleo diet for women is based on what we likely were eating during the majority of our evolution, which is, basically, meat and vegetables. Notice how we have molar teeth for grinding? That means we are meant to eat vegetables. And, our canine teeth? For eating meat: its simple structure determining function. If we had four stomachs like cows, I’d say we’re designed to eat grass and raw vegetables but we don’t so eat your vegetables either fermented or cooked.

Animals that eat lots of plant matter and little protein have several stomachs that allow them to ferment the plant matter in their body. Humans have found a way to do this using glass jars (ie sauerkraut or kombucha).

For women after menopause, hormone replacement therapy can certainly be life enhancing. The decline in hormone production after menopause can be life changing.

In younger women, progesterone begins declining around age 30. This decline is similar to the decline in testosterone that men experience starting around age 30 years old. The decline of progesterone in women is linked to decline in libido, problems sleeping, anxiety, and other health problems. During times of stress the body will “steal” hormone precursors to make stress hormones like cortisol. This will lead to weight gain and thyroid problems because cortisol effects blood sugar regulation and decreases thyroid hormone levels.

“The TSH test should not be utilized in the diagnostic confirmation of hypothyroidism because the diagnostic accuracy of the TSH test in confirming hypothyroidism is unknown, several aspects of the TSH test indicate its poor diagnostic utility and thyroid hormone trial therapy is the best method for achieving diagnostic certainty.”2

Before menopause, progesterone and thryoid hormone deficiencies are quite common. Subclinical hypothyroidism, for example, is low thyroid that is missed by standard lab testing. Most doctors merely check the signal from the brain to the thyroid, called the TSH test. The TSH test has been shown to be almost worthless unless there is a sever problem because most people are going to fall into the gray area.

In a study of adult taiwanese in 2012, researchers found that subclinical hypothyroidism (low thyroid that doesn’t meet traditional criteria) were at increased risk of death by ALL causes including increased cardiovascular risk.3 That means if you have low thyroid that is missed by a TSH lab test your at increased risk of death. At Femme Clinique we do comprehensive thyroid lab testing. We also looks for symptoms and physical signs of low thyroid function. We believe in treating people not labs. Lab testing is merely a guide and there are many things that we can measure but many things that we cannot measure presently.

In the near future, 3-4 years, we will be able to determine your entire genetic code and look for small nuclear polymorphisms (SNPs) that may be limiting your potential. These SNPs are small pieces of genetic code that vary from individual and certain versions can cause greater needs for certain hormones and nutrients.

In addition, the environment can affect how our bodies function and there is evidence that birth control pills can cause long lasting changes in liver function even after discontinuing the pill. Birth control pills can cause an increase in proteins produced by the liver that bind hormones in your blood and prevent them from being active in your body. The good news is there are ways to address this problem and we can measure these levels with a simple blood draw.
